Transition of Care Division at Alfred I. duPont Hospital for Children

Your child will need to find new physicians who treat adults by the time he/she is 18 to 21 years of age. The Transition of Care Division is a program that helps patients at the AI duPont Hospital to create a smooth health care transition.

Request a consultation with the Division of Transition of Care, ideally when you child is approximately 16 years of age.  Your child’s physicians can request a consultation, or you or your adolescent child can make the request.
